CommuniCare Health Centers is a Federally Qualified Health Center providing high quality health care to low-income, uninsured and underinsured residents of Yolo County since 1972. CommuniCare offers comprehensive primary medical and dental services, perinatal services, substance abuse treatment and behavioral health services, health education and health coverage enrollment services through five community clinic sites and outreach programs.
